On or about December 18 and/or 24, 2001, at or near 1001 Derby Lane, Westchester, Illinois, while off duty, committed the offense of Attempted Deceptive Practices, in that she submitted a physically altered Illinois State lottery ticket, attempting to cause the Illinois State Lottery to incur a pecuniary obligation of $5000.00, in violation of Chapter 720 ILCS 5/17- 1(B)(a) and/or Chapter 720 ILCS 5/8-4(a);

On or about December 18 and/or 24, 2001, at or near 1001 Derby Lane, Westchester, Illinois, while off duty, committed the offense of Attempted Deceptive Practices, in that she submitted a physically altered Illinois State lottery ticket, attempting to cause the Illinois State Lottery to incur a pecuniary obligation of $2500.00, in violation of Chapter 720 ILCS 5/17- 1(B)(a) and/or Chapter 720 ILCS 5/8-4(a);

On or about December 18 and/or 24, 2001, at or near 1001 Derby Lane, Westchester, Illinois, while off duty, committed the offense of Attempted Deceptive Practices, in that she, with intent to defraud, submitted a physically altered Illinois State lottery ticket, attempting to cause the Illinois State Lottery to incur a pecuniary obligation of $5000.00, in violation of Chapter 20 ILCS 1605/14.2;

On or about December 18 and/or 24, 2001, at or near 1001 Derby Lane, Westchester, Illinois, while off duty, committed the offense of Attempted Deceptive Practices, in that she, with intent to defraud, submitted a physically altered Illinois State lottery ticket, attempting to cause the Illinois State Lottery to incur a pecuniary obligation of $2500.00, in violation of Chapter 20 ILCS 1605/14.2.

On or about December 18 and/or 24, 2001, at or near 1001 Derby Lane, Westchester, Illinois, while off duty, brought discredit upon the department and impeded the Department's efforts to achieve its policy and goals in that she among submitted a physically altered Illinois State lottery ticket, attempting to cause the Illinois State Lottery to incur a pecuniary obligation of $5000.00, in violation of Chapter 720 ILCS 5/17-1(B)(a) and/or Chapter 720 ILCS 5/8-4(a);
