Eric Ruiz

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, without lawful authority, he displayed and/or pointed an unregistered firearm at Roig I. Maya, a minor, placing him in reasonable apprehension of receiving a battery, in violation of Illinois Compiled Statutes Chapter 720 Section 5/12-2(a);

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, without lawful authority, he displayed and /or pointed an unregistered firearm at Dale J. Sopoci, placing him in reasonable apprehension of receiving a battery, in violation of Illinois Compiled Statutes Chapter 720 Section 5/12-2(a);

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, without lawful authority, he displayed and/or pointed an unregistered firearm at Roig I. Maya, a minor, thereby engaging in disorderly conduct by unreasonably alarming or disturbing another and/or provoking a breach of the peace, in violation of Illinois Compiled Statutes Chapter 720 Section 5/26-1(a) (1);

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, without lawful authority, he displayed and/or pointed an unregistered firearm at Dale J. Sopoci, thereby engaging in disorderly conduct by unreasonably alarming or disturbing and/or provoking a breach of peace, in violation of Illinois Compiled Statutes Chapter 720 Section 5/26-1(a) (1);

From on or about March 14, 1989 to the present, or for some time therein, at or near 2200 N. Merrimac, Chicago, Illinois, he failed to register a firearm in his possession, namely a Smith & Wesson, Model 60, .38 caliber 5 shot revolver, Serial Number BBW5610, in violation of Chapter 8 of the Chicago Municipal Code, Section 20-040.

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, while off duty, he displayed and/or pointed an unregistered firearm, without lawful authority, at Roig I. Maya, a minor, placing him in reasonable apprehension of receiving a battery, in violation of Illinois Compiled Statutes 720 Section 5/12-2(a);

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, he displayed and/or pointed an unregistered firearm, without lawful authority, at Dale J. Sopoci, placing him in reasonable apprehension of receiving a battery, in violation of Illinois Compiled Statutes Chapter 720 Section 5/12-2(a)

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, he, among other things, displayed and/or pointed an unregistered firearm without lawful authority at Roig I. Maya, a minor thereby unreasonably provoking a breach of the peace, in violation of Illinois Compiled Statutes Chapter 720 Section 5/26-1(a)(1);

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, he, among other things, displayed and/or pointed an unregistered firearm without lawful authority at Dale J. Sopoci, thereby unreasonably provoking a breach of the peace, in violation of Illinois Compiled Statutes Chapter 720 Section 5/26-1(a)(1);

From on or about March 14, 1989 to the present, or for some time therein, at or near 2200 N. On or about June 26, 2002, he was found guilty of one and/or more counts of disorderly conduct, Illinois Compiled Statutes Chapter 720 Section 5/26-1(a)(1), by the Circuit Court of the 15th Judicial District, located in Lee County, Illinois.

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, he consumed alcoholic beverages while in possession of an unregistered firearm, a Smith & Wesson, Model 60, .38 caliber 5 shot revolver, Serial Number BBW5610, thereby violating General Order 92-3, Article III, Section C.

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, without lawful authority, he displayed and/or pointed an unregistered firearm at Roig I. Maya, a minor;

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, without lawful authority, he displayed and/or pointed an unregistered firearm at Dale J. Sopoci.

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, he refused to cooperate with one and/or more of the Lee County Sheriff's Department, and/or members of the Woodhaven Lakes Association;

On or about September 1, 2001, at approximately 20:30 hours, at or near Woodhaven Lakes Association, 507 LaMoille Road, Sublette, Illinois, while off duty, he directed one and/or more profanities towards one and/or more of the Lee County Sheriff's Department, and/or members of the public.
