Southwest Highway, Oaklawn, IL. and failed to register the firearm with the City of Chicago, thereby violating Municipal Code of Chicago Chapter § 8-20-040(a).

On or about November 5, 1996, Civilian Benjamin Dease committed the offense of failure to register a firearm, in that he purchased a 9MM Smith and Wesson model #669 handgun, serial number TBR1317 from Chicago Ridge Gun Shop & Range, located at or near 10349 Southwest Highway, Chicago Ridge, IL. and failed to register the firearm with the City of Chicago, thereby violating Municipal Code of Chicago Chapter § 8-20-040(a).

On or about November 19, 1996, Civilian Benjamin Dease committed the offense of owning a fireaini used illegally, in that he owned an unregistered 9MM Smith and Wesson model #9155 handgun, serial number VBK5176 that was unlawfully possessed by Keyairre Hilderbrand, whom was charged with Unlawful Use of a Weapon in violation of Chapter 720, Act 5, Section 24-1 (a)(1) of the Illinois Compiled Statutes and No Firearm Owner's Identification Card in violation of Chapter 430, Act 65, Section 2 (a)(1) of the Illinois Compiled Statutes, thereby violating Municipal Code of Chicago Chapter § 8-20-241.

On or about July 27, 1998, Civilian Benjamin Dease committed the offense of owning a firearm used illegally, in that he owned an unregistered 9MM Bryco model #J59 handgun, serial number 1051354 that was unlawfully stored in the vehicle of Daniel McShan whom was charged with Unauthorized Manufacture or Delivery in violation of Chapter 720, Act 570, Section 401 of the Illinois Compiled Statutes, thereby violating Municipal Code of Chicago Chapter § 8-20-241.

On or about November 14, 2002, Civilian Benjamin Dease made a false statement in response to the question why he failed to register four handguns described as one 9MM Smith and Wesson model #669 handgun, serial number TBR1317, one 9MM Bryco model #J59 handgun, serial number 1051354, one 9MM Bryco model #J59 handgun, serial number 1048629, and one 9MM Smith and Wesson model #9155 handgun, serial number VBK5176, in that he stated that he was unaware of the gun registration laws when he previously admitted that he knew he could not register the handguns in the City of Chicago on April 15, 2002.
