On or about February 9, 2003, at or near 9129 S. Clyde, Chicago, you failed to fulfill your duties as an animal owner, in that you failed to provide one or more of your dogs with accessible food and/or clean water and/or you failed to provide said dogs with humane care and treatment, and failed to obtain veterinary care for one or more of the dogs when it was needed to prevent their suffering, in violation of Chapter 510 ILCS 70/3(a),(c), and (d).

On or about February 9, 2003, at or near 9129 S. Clyde, Chicago, you engaged in the cruel treatment of animals, in that you kept one or more dogs in unsanitary makeshift kennels without access to food and/or water in areas surrounded by piles of fecal matter, and where one or more of said dogs was in a starved condition and/or had open wounds on its body, in violation of Chapter 510 ILCS 70/3.01.

On or about February 9, 2003, at or near 9129 S. Clyde, Chicago, you intentionally committed an act that caused one or more of the dogs under your care to suffer serious injury, in that you kept one or more dogs in
unsanitary makeshift kennels without access to food and/or water in areas surrounded by piles of fecal matter, and where one or more of said dogs was in a starved condition and/or had open wounds on its body, in violation of Chapter 510 ILCS 70/3.02.

On or about February 9, 2003, at or near 9129 S. Clyde, Chicago, you intentionally committed an act that caused the death of a dog under your care, in that you kept the dog in unsanitary conditions among feces and urine,
without access to food or clean water causing the dog to be in a condition of starvation and/or allowed the dog to receive injury and open wounds thereby causing the dog's death, in violation of Chapter 510 ILCS 70/3.02.

On or about February 9, 2003, at or near 9129 s. Clyde, Chicago, you failed to provide one or more dogs under your care with accessible food and/or clean water and/or appropriate veterinary care and/or sanitary surroundings resulting in the starvation, injury, and/or death of one or more of the dogs.
