On or about June 1, 2001, she disobeyed an order given by Sergeant Daniel Simon, when she failed to report to Area Two Firearms Range for mandatory Firearms Training Program, in violation of Department Special Order 01-02, Section II.A., and/or Section III. D and/or Department Special Order 01-02-03 Section III.

On or about June 11, 2001, she disobeyed an order given by Sergeant Daniel Simon, when she failed to report to the Medical Services Section.

On or about June 12, 2001, she disobeyed an order given by Sergeant Daniel Simon, when she failed to report to the Medical Services Section.

On or about June 14, 2001 through June 21, 2001, or some period of time therein, she failed to follow procedures for requesting a Leave of Absence, thereby violating General Order 94-05-07, Section I.A.

On or about June 2001, she failed to telephone or report to the Medical Services Section on the second day of a medical absence, as required by General Order 98-02-02A, Section III

On or about June 8, 2001 through June 13, 2001, on one or more occasions, she was absent without appropriate authorization and/or permission.
