On or about March 11, 2003, at around 4 p.m., at or near Skokie, Illinois, while on duty, Dexter Quade used his position as a police officer for personal gain or influence in that he, while driving a marked squad car, activated emergency lights and/or siren and stopped the vehicle of Debra Harrison, thus impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department;

On or about March 11, 2003, at around 4 p.m., at or near Skokie, Illinois, while on duty, Dexter Quade followed the vehicle of Debra Harrison outside of the 16th District boundaries into Skokie, Illinois, without proper authorization, thus impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department;

On or about March 23, 2003, at or near 6355 North Central, Dexter Quade attempted to impede the Office of Professional Standards (OP S) investigation by asking Carol Anichini to tell Debra Harrison not to talk to OPS and/or offered Anichini money, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department;

On or about March 23, 2003, at or near 6355 North Central, Dexter Quade asked Carol Anichini to tell Debra Harrison to rip up the fake parking ticket Quade had issued Harrison and/or otherwise asked Anichini to tell Harrison not to tell OPS about a false parking ticket, thereby impeding the Department's efforts to achieve
its policy and goals and/or bringing discredit upon the Department;

On or about March 23, 2003, at or near 6355 North Central, Dexter Quade disobeyed a direct order given to him by Captain Roger Whalen on or about March 12, 2003 in that he asked Carol Anichini to contact Debra Harrison on his behalf, thereby impeding the Departments efforts to achieve its policy and goals and/or bringing discredit upon the Department;

On or about September 5, 2001, Dexter Quade issued Debra Harrison a false parking ticket, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department;
