Shelisa Jones

There was insufficient evidence presented by the Superintendent to prove that on or about December 5, 2002, in the vicinity of 8645 South Blackstone
Avenue, Chicago, Police Officer Shelisa Jones, while off-duty, shot Jerome Michael Sajna without justification.

On or about December 5, 2002, Police Officer Shelisa Jones, during one or more interviews with Chicago Police Department personnel, provided a false account regarding one or more phone calls that she made
to the Office of Emergency Communications.

There was insufficient evidence presented by the Superintendent to prove that on or about March 26, 2003, during a statement at the Office of Professional Standards, Police Officer Shelisa Jones provided a false account of the circumstances surrounding the shooting of Jerome Michael Sajna on December 5, 2002.

On or about December 5, 2002, Police Officer Shelisa Jones, during one or more interviews with Chicago Police Department personnel, provided a false account regarding one or more phone calls that she made to the Office of Emergency Communications
