Ryan Milot

That on or about February 3, 2000, P.O. Milot, Star #18252, at the Education and Training Division, r d floor, 1301 West Adams, was to undergo a mandatory Fitness for Duty evaluation which was to include a psychiatric evaluation, a complete physical examination and a urinalysis and, subsequent to an explanation of the process, he disobeyed orders to remain and failed to cooperate in the
process, thereby impeding the Department's efforts to achieve its policy and goals or brings discredit upon the Department.

That on or about February 3, 2000, P.O. Milot, Star #18252, at the
Education and Training Division, 2nd floor, 1301 West Adams, was to undergo a mandatory Fitness for Duty evaluation which was to include a urinalysis and, during the explanation of the process, he was given a direct order not to leave the office and failed to comply with that order in that he exited the office, thereby disobeying an order or directive whether written or oral.

That on or about February 3, 2000, P.O. Milot, Star #18252, at the
Education and Training Division, 2nd floor, 1301 West Adams, was to undergo a mandatory Fitness for Duty evaluation which was to include a urinalysis, but he exited the office and was given a direct order to return to the office but he failed to comply with that order, thereby disobeying an order or directive whether written or oral.
