On or about January 2004, Police Officer Klups provided false information to Sergeant Christopher P. Suber, Star 2660, and/or on his Unit Personnel Card, when he reported that his home telephone number was (773) 881- 9982; thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

Klups failed to follow Medical Roll procedures when he failed to contact an on-duty supervisor in his assigned unit or detail, and to provide information required to complete the Sickness Report/Absence from Home Log, while on the medical roll under the classification of
Stationary Recuperation; thereby impeding the Department's efforts
to achieve its policy and goals or bringing discredit upon the
Department.
