Gregory Klups

On or about January 2004, Police Officer Klups provided false information to Sergeant Christopher P. Suber, Star 2660, and/or on his Unit Personnel Card, when he reported that his home telephone number was (773) 881- 9982; thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

In or about January 2004, Police Officer Klups provided false information to Sergeant Christopher P. Suber, Star 2660, and/or on his Unit Personnel Card, when he reported that his home telephone number was (773) 885-9275; thereby impeding the Department's
efforts to achieve its policy and goals or bringing discredit upon the Department.

From on or about February 23, 2004, through on or about March 15, 2004, or on one or more dates therein, Police Officer Gregory J. Klups failed to follow Medical Roll procedures when he failed to be at his declared residence, 3311 W. 108th Street, Chicago, Illinois, while on the medical roll under the classification of Stationary Recuperation; thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

From on or about February 23, 2004, through on or about March 15, 2004, or on one or more dates therein, Police Officer Gregory J. Klups failed to follow Medical Roll procedures when he failed to contact an on-duty supervisor in his assigned unit or detail, and to provide information required to complete the Sickness Report/Absence from Home Log, while on the medical roll under the classification of
Stationary Recuperation; thereby impeding the Department's efforts
to achieve its policy and goals or bringing discredit upon the
Department.

From on or about February 23, 2004, through on or about March 15, 2004, or on one or more dates therein, Police Officer Gregory J. Klups failed to follow Medical Roll procedures when he failed to contact an on-duty supervisor in his assigned unit or detail, and to provide information required to complete the Sickness Report/Absence from Home Log, while on the medical roll under the classification of
Stationary Recuperation; thereby impeding the Department's efforts
to achieve its policy and goals or bringing discredit upon the
Department.
On or about March 15, 2004, Police Officer Klups disobeyed a
direct order given by Sergeant Suber, Star 2660, to report to his
unit to sign SPAR number 04-02-0553; thereby impeding the
Department's efforts to achieve its policy and goals or bringing
discredit upon the Department.

On or about March 15, 2004, Police Officer Klups disobeyed a direct order given by Sergeant Suber, Star 2660, to report to his unit to sign SPAR number 04-02-0553; thereby disobeying an order or directive, whether written or oral.

In or about January 2004, Police Officer Klups provided false information to Sergeant Christopher P. Suber, Star 2660, and/or on his Unit Personnel Card, when he reported that his home telephone number was (773) 881- 9982; thereby making a false report, written or oral.

In or about January 2004, Police Officer Klups provided false information to Sergeant Christopher P. Suber, Star 2660, and/or on his Unit Personnel Card, when he reported that his home telephone number was (773) 885-9275; thereby making a false report, written or oral.

On or about April 8, 2004, Police Officer Gregory J. Klups provided false information to Police Agent Kaye Calderaro, Star 17655, of the Internal Affairs Division, regarding his absence from his residence on March 15, 2004; thereby making a false report, written or oral.

Klups failed to follow Medical Roll procedures when he failed to be at his declared residence, 3311 W. 108th Street, Chicago, Illinois, while on the medical roll under the classification of Stationary Recuperation, as required by General Order 98-02.III.F; thereby failing to follow the Department's medical roll procedures.
