Christopher Berlanga

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer l3erlanga, at least 21 years of age, caused the death of a person, William Alvarez, while operating a motor vehicle with an alcohol concentration equivalent to at least 0.15 grams of alcohol per 100 milliliters of his blood, in violation of Indiana Code 9-30-5-5 (b) (1) (A).

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga caused the death of a person, William Alvarez, while operating a motor vehicle while intoxicated, in violation of Indiana Code 9-30-5-5 (a) (3).

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga caused serious bodily injury to a person, Thomas A. Miller, while operating a motor vehicle with an alcohol concentration equivalent to at least 0.08 grams of alcohol per 100 milliliters of his blood, in violation of Indiana Code 9-30-5-4 (a) (1) (A).

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga caused serious bodily injury to a person, Thomas A. Miller, while operating a motor vehicle while intoxicated, in violation of Indiana Code 9-30-5-4 (a) (3).

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga caused the death of William Alvarez while operating a motor vehicle with an alcohol concentration equivalent to at least 0.15 grams of alcohol per 100 milliliters of his blood, and/or while intoxicated, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga caused serious bodily injury to Thomas A. Miller while operating a motor vehicle with an alcohol concentration equivalent to at least 0.08 grams of alcohol per 100 milliliters of his blood, and/or while intoxicated, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga was intoxicated, thereby impeding the Department's efforts to achieve its policy and goals and/ or bringing discredit upon the Department.

On or about November 21, 2006, in the Office of the Internal Affairs
Division, Sergeant James Cradick gave you a direct order to respond to
questions asked of you as part of an Internal Affairs Division
investigation, and you refused to answer, thereby disobeying an order or
directive, whether written or oral.

On or about January 16, 2006, in the vicinity of Indianapolis Boulevard and Martha Street, Highland, Indiana, Officer Berlanga was intoxicated on or off duty in that his blood alcohol content was 211 mg/ dL or .21 grams of alcohol per 100 milliliters of his blood.
