From on or about June 22, 2004, through on or about the present date, or for some period of time therein, you resided at 3038 Elliot Lane, Homewood, Illinois; you thereby engaged in conduct prohibited by Title 2, Chapter 152, Section 340 of the Municipal Code of Chicago.

From on or about April 4, 2006 through on or about June 9, 2006, or for some period of time therein, you displayed and/or affixed
Illinois license plate No. JEZZ on and/or to a 2000 Honda, which
license plate was not authorized for use on such vehicle, in
violation of Section 625 ILCS 5/4-104(a)(4) and/or Section 625 ILCS
5/3-703.

From on or about June 22, 2004, through on or about the present date, or for some period of time therein, you resided at 3038 Elliot Lane, Homewood, Illinois, thereby impeding the Department's
efforts to achieve its policy and goals and/or bringing discredit
upon the Department.

From on or about June 22, 2004, through on or about the present date, or for some period of time therein, you resided at 3038 Elliot Lane, Homewood, Illinois, thereby impeding the Department's
efforts to achieve its policy and goals and/or bringing discredit
upon the Department. From on or about April 4, 2006 through on or about June 9, 2006, you displayed and/or affixed Illinois license plate No. JEZZ on and/or to a 2000 Honda, which license plate was not
authorized for use on such vehicle, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the
Department.

From on or about June 22, 2004, through on or about the present date, or for some period of time therein, you resided at 3038 Elliot Lane,
Homewood, Illinois; thereby failing to live within the corporate
boundaries of the City of Chicago.

From on or about June 22, 2004, through on or about the present date, or during some period of time therein, you resided at 3038 Elliot Lane,
Homewood, Illinois, however, your Employee Change of Address Form
dated on or about April 20, 2005, declared your old address as 5131 S.
Dorchester and your new address as 5493 S. Cornell, Chicago, Illinois,
thereby failing to provide the Department with a current address and/or
telephone number.
