On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he discharged his weapon while intoxicated.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he failed to make the required notifications
regarding discharge of his weapon.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he engaged in an unjustified altercation
with the victim, Diana Poggy.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he threatened to commit suicide.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he possessed his firearm while consuming
alcoholic beverages.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he possessed one or more unregistered firearms.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he placed his gun into his mouth and/or relinquished his firearm to victim Diana Poggy and/or told her to shoot him.

On or about February 25, 2000, at
approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he disobeyed the provisions of General Order 86-8, III, A, in that he discharged his firearm one or more times when the use of such force was not necessary.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he disobeyed the provisions of General Order 99-01, Section V, A (a) (d), in that he failed
to immediately notify the Office of Emergency
Communications, or the District of the occurrence.

On or about February 25, 2000, at
approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he disobeyed the provisions of General Order 00-01, Section V, A (f), in that he failed to
complete a Weapons Discharge Report or To-From Subject Report, after discharging his weapon at said location.

On or about February 25, 2000, at
approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he disobeyed the provisions of General Order 92-3 Addendum 5B, Section IV, B, in that he
was found to possess one or more unregistered firearms.

On or about February 25, 2000, prior to 1245
1250 hours, at or about "My Mistake" bar located at or about 23 rd and Damen, while off duty, he disobeyed provisions of
General Order 92-3, Section III, C, in that he consumed alcoholic beverages while in possession of his firearm.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 N. Albany, second floor, while off duty, he pulled Diana Poggy from her bed and/or grabbed her by the arms, and/or pushed her.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he was intoxicated.

On or about February 25, 2000, at
approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he failed to make an oral report to the desk sergeant at the District of the occurrence and failed to follow such oral report with a written report on the prescribed form, after discharging his weapon.

On or about February 25, 2000, at approximately 1330 hours, at or about 3812 North Albany, second floor, while off duty, he unnecessarily discharged his weapon one
or more times.
