From July 14, 1998, to the present, or for some period of time therein, she
failed to notify the Department of her address when she ceased residing
within the corporate boundaries of the City of Chicago, thereby failing to
provide the Department with a current address and phone number.

From July 14, 1998, to the present, or for some period of time therein, she
failed to reside within the corporate boundaries of the City of Chicago.

On or about January 21, 1999, she stated that she resided within the
corporate boundaries of the City of Chicago, when in fact she did not,
thereby giving a false report.

From July 14, 1998, to the present, or for some period of time therein, she
failed to submit "Personnel Change Notice" and/or change of address forms
to the Chicago Police Department with regard to her change of address and
/or telephone number when she ceased residing at 737 North Mayfield,
Chicago, Illinois, thereby disobeying General order 94-5.

On or about January 21, 1999, she falsely stated that she resided within the
corporate boundaries of the City of Chicago, when in fact she did not,
thereby impeding the Department's efforts to achieve its policy and goals
and/or bringing discredit to the Department.

From July 14, 1998 to the present, or for some period of time therein, she
failed to reside within the corporate boundaries of the City of Chicago,
thereby impeding the Department's efforts to achieve its policy and goals
and/or bringing discredit upon the Department.

From July 14, 1998, to the present, or for some period of time therein, she
failed to reside within the corporate boundaries of the City of Chicago, in
violation of Title 2, Chapter 27, Section 340 of the Municipal Code of
Chicago.
