On or about January 4, 2013, at approximately 1450 hours, at or nearing the intersection of Street and Central Avenue in Chicago Ridge, Illinois, Police Officer William J. Donovan, while driving a vehicle northbound on Central Avenue in Chicago Ridge, Illinois, failed to yield the right-of-way when turning left within an intersection, in violation of 625 ILCS 5/1 1-902.

On or about January 4, 2013, at approximately 1450 hours, at or about 11100 South Central Avenue in Chicago Ridge, Illinois, Police Officer William J. Donovan, while off-duty, drove a vehicle that got involved in a motor vehicle accident resulting in damage to a vehicle and did not remain at the scene of the accident until the requirements of 625 ILCS 5/1 1-403 (Duty to give information and render aid) were fulfilled, in violation of 625 ILCS 5/11-402.

Donovan, pled guilty to and was found guilty of failure to yield the right-of-way required when approaching and/or entering an intersection and/or when turning left in the Circuit Court of Cook County, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

On or about January 4, 2013, at approximately 1506 hours, at or near the intersection of 111th Street and Central Avenue in Chicago Ridge, Illinois, Police Officer William J. Donovan was arrested by the Chicago Ridge Police Department, but failed to immediately submit a written report to the Chicago Police Department and/or failed to notify the Chicago Police Department, until on or about January 5, 2013, approximately sixteen (16) hours after his arrest, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.
