Theron Jones

On or about March 26, 2009, at approximately 7:30 PM, at or near the M&MMini Mart, A.K.A. the Burham Quick Food Mart, located at 1201 North Austin Boulevard, Officer Jones knowingly and without legal authority detained Jeremy Augustus, in violation of 720 1LCS 5/10-3 ("unlawful restraint"), thereby violating a law or ordinance.

On or about March 26, 2009, at approximately 7:30 PM, at or near the M&MMini Mart, A.K.A. the Burham Quick Food Mart located at 1201 North Austin Boulevard, Officer Jones detained and/or falsely arrested Jeremy Augustus, in violation of Fourth Amendment of the Constitution of the United States of America, thereby violating a law or ordinance.

On or about March26, 2009, at approximately 7:30 PM,at or near the M&M Mini Mart, A.K.A. the Burham Quick Food Mart, located at 1201North Austin Boulevard, Officer Jones pushed and/or conducted a takedown on Jeremy Augustus without justification, thereby impeding the Department's efforts to achieve its policy and goals, and/or bringing discredit upon the Department.

On or about March 26, 2009, at approximately 7:30 PM,at or near the M&M Mini Mart, A.K.A. the Burham Quick Food Mart, located at 1201North Austin Boulevard, Officer Jones knowingly and without legal authority detained Jeremy Augustus, and/or falsely arrested Jeremy Augustus, thereby impeding the Department's efforts to achieve its policy and goals, and/or bringing discredit uponthe Department.

On or about March26, 2009, Officer Jones falsified an arrest report by falsely stating that an employee of the Burham Quick Food Mart informed himthat Jeremy Augustus "was given notice and ask [sic] not to return to the store after he caused a disturbance being loud, disruptive, exhibited rude behavior using vulgar profanity while in the store and in presents [s/c] of other customers," or used words to that effect, and/or that Jeremy Augustus was arrested for trespassing, or used words to that effect, thereby impeding the Department's efforts to achieve its policy and goals, and/or bringing discredit upon the Department.

On or about February 24, 2011, at approximately 12:30PM,during a statement with the Independent Police Review Authority, Officer Jones made one or more of the following false statements: that he initially stopped Jeremy Augustus because he had double parked his car, or used words to that effect; and/or that Jeremy Augustus was bouncing off the walls and/or hitting his head against the wall, or used words to that effect; and/or that Jeremy Augustus was arrested for trespassing, or used words to that effect; and/or that he did not falsify the arrest report of Jeremy Augustus, or used words to that effect, thereby impeding the Department's efforts to achieve its policy and goals, and/or bringing discredit upon the Department

On or about March26, 2009, at approximately 7:30 PM,at or near the M&MMini Mart, A.K.A. the Burham Quick FoodMart, located at 1201 North Austin Boulevard, Officer Jones pushed and/or conducted a takedown on Jeremy Augustus and failed to complete a Tactical Response Report, in violation of General Order G03-02-05, ("Incidents Requiring the Completion of a Tactical Response Report", formerly General Order G02-08-05), thereby impeding the Department's efforts to achieve its policy and goals, and/or bringing discredit upon the Department.

On or about March2 6, 2009, at approximately 7:30 PM,at or near the M&M Mini Mart, A.K.A. the Burham Quick Food Mart, located at 1201 North Austin Boulevard, Officer Jones pushed and/or conducted a takedown on Jeremy Augustus and failed to complete a Tactical Response Report, in violation of General Order G03-02-05, ("Incidents Requiring the Completion of a Response Report", formerly General Order G02-08-05), thereby disobeying an order or directive, whether written or oral.

On or about March 26, 2009, at approximately 7:30 PM,at or near the M&M Mini Mart, A.K.A. the Burham Quick Food Mart, located at 1201NorthAustin Boulevard, Officer Jones pushed and/or conducted a takedown on Jeremy Augustus without justification and/or knowingly and without legal authority detained and/or falsely arrested Jeremy Augustus, thereby disrespecting or maltreating any person, while on or off duty.

On or about March 26, 2009, at approximately 7:30 PM,at or near the M&M Mini Mart, A.K.A. the Burham Quick FoodMart, located at 1201North Austin Boulevard, Officer Jones pushed and/or conducted a takedown on Jeremy Augustus without justification, thereby engaging in an unjustified verbal or physical altercation with any person, while on or off duty.
