In that in or about June 2000, on one or more occasions, she committed the offense of Obtaining Control Over Card As Security For Debt With Intent To Defraud, in that she contacted one or more hotels in Chicago, Illinois, and reserved one or more rooms for one or more nights in or about June 2000, using Derek Rhodes' credit/debit card, without his consent, resulting in a charge to his account, thereby violating 720 ILCS 250/7. In that in or about June 2000, on one or more occasions, she committed the offense of Obtaining Control Over Card As Security For Debt With Intent To Defraud, in that she contacted one or more hotels in Chicago, Illinois, and reserved one or more rooms for one or more nights in or about June 2000, using Derek Rhodes' credit/debit card, without his consent, resulting in a charge to his account, thereby violating 720 ILCS 250/7.

In that in or about August 2000, she failed to immediately submit a report to her commanding officer when she was notified by a member of a Police Department other than the Chicago Police Department, that she was a suspect, and/or under investigation, in an investigation being conducted by that police department, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

In that on or about August 27, 2000, having been advised by a member of a Police Department other than the Chicago Police Department, that she was a suspect in an investigation, and/or was under investigation, appeared at the office of that police department, other than the Chicago Police Department, was read her Miranda rights and gave a statement that was videotaped with her knowledge and subsequently, failed to immediately submit a report to her commanding officer, thereby violating the provisions of General Order 93-3, Addendum 5, IV-A.

On or about February 20, 2000, Officer Robert Niziolek submitted to a Breath Alcohol Test which results revealed that he had a blood alcohol level of approximately 0.225%, thus being intoxicated while off duty.
