On or about January 27, 2000, at or near 11124 South Campbell Avenue and off duty, Officer Niziolek maltreated his former wife, Cathleen A. Niziolek, in that he struck and/or forcefully squeezed her hand and/or arm; and/or pulled her hair, causing and/or threatening to cause bodily harm to Ms. Niziolek in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-1 (Assault), and/or 720 ILCS 5/12-5 (Reckless Conduct), 720 ILCS 5/12-3 (Battery), and/or 720 ILCS 5/12-3.2 (Domestic Battery).

On or about February 20, 2000, at or near 11124 South Campbell Avenue and off duty, Officer Niziolek maltreated his former wife, Cathleen A. Niziolek, in that he shoved her, grabbed her hair and/or struck her one or more times on her head, face and/or back thus causing and/or threatening to cause bodily harm to Ms. Niziolek in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-1 (Assault), and/or 720 ILCS 5/12-5 (Reckless Conduct), 720 ILCS 5/12-3 (Battery), and/or 720 ILCS 5/12-3.2 (Domestic Battery). On or about February 20, 2000, at or near 11124 South Campbell Avenue and off duty, Officer Niziolek maltreated his former wife, Cathleen A. Niziolek, in that he shoved her, grabbed her hair and/or struck her one or more times on her head, face and/or back thus causing and/or threatening to cause bodily harm to Ms. Niziolek in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-1 (Assault), and/or 720 ILCS 5/12-5 (Reckless Conduct), 720 ILCS 5/12-3 (Battery), and/or 720 ILCS 5/12-3.2 (Domestic Battery).

Niziolek, in that he threw her against a television set thus causing and/or threatening to cause bodily harm to Ms. Niziolek in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-1 (Assault), and/or 720 ILCS 5/12-5 (Reckless Conduct), 720 ILCS 5/12-3 (Battery), and/or 720 ILCS 5/12-3.2 (Domestic Battery). On or about April 7, 2000, at or near 11124 South Campbell Avenue and off duty, Officer Niziolek maltreated his former wife, Cathleen A. Niziolek, in that he threw her against a television set thus causing and/or threatening to cause bodily harm to Ms. Niziolek in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-1 (Assault), and/or 720 ILCS 5/12-5 (Reckless Conduct), 720 ILCS 5/12-3 (Battery), and/or 720 ILCS 5/12-3.2 (Domestic Battery).
