- On or about June 25, 2015, at or about 3510 S. Michigan Avenue, in Chicago, Officer Belaschky falsely reported to Sergeant Sean Rice that Dr. Elias's answering machine had told him that his appointment on about May 5, 2015, was cancelled because Dr. Elias was in surgery, when Dr. Elias did not provide such information on an answering machine, thereby making a false report, written or oral.

- On or about May 8, 2015, via telephone, Officer Belaschky falsely reported to Sergeant Thomas Risley that his appointment with Dr. Elias on about May 7, 2015, was cancelled because Dr. Elias was in surgery, when said appointment had not been cancelled because Dr. Elias was in surgery, thereby making a false report, written or oral.

- On or about May 6, 2015, via telephone, Officer Belaschky falsely reported to Sergeant Thomas Risley that he had been referred by Dr. Richard Hayes to an orthopedic surgeon, when Dr. Hayes had not made such a referral; and/or that his appointment with said orthopedic surgeon, Dr. George Elias, on about May 5, 2015, was cancelled because Dr. Elias was in surgery, when said appointment had not been cancelled because Dr. Elias was in surgery, thereby making a false report, written or oral.
