Adm. Code 1240.80(a) (1999)), and/or the Driver's Privacy Protection Act of 1994, (18 U.S.C. §§ 2721-2722 (2012)), and/or CPD departmental policies and procedures.

On or about September 16, 2013, Officer Sanchez violated section 2721 and/or section 2722 of the Driver's Privacy Protection Act of 1994, (18 U.S.C. §§ 2721-2722 (2012)), when he knowingly obtained personal information from the motor vehicle records of one or more individuals associated with and/or previously associated with [redacted] by conducting one or more LEADS inquiries on the names and/or personal information of said individuals without an official police purpose and/or for personal purposes and/or for an unpermitted use.

On or about September 16, 2013, Officer Sanchez violated section 1240.80(a) of the Illinois Administrative Code, (20 Ill. Adm. Code 1240.80(a) (1999)), entitled "Dissemination of Data Obtained through LEADS," when he conducted one or more Law Enforcement Agencies Data System ("LEADS") inquiries on the names and/or personal information of one or more individuals associated with and/or previously associated with [redacted] without an official police purpose and/or for personal purposes.

[redacted], No. 2015 D [redacted], in which he averred that "[redacted] has never been harassed, stalked, abused in any shape or form by MICHAEL [Sanchez]," a false statement material to an issue or point in question that he did not believe to be true but that he certified as true and correct pursuant to section 1-109 of the Illinois Code of Civil Procedure.

On or about May 21, 2015, Officer Sanchez violated section 1-109 of the Illinois Code of Civil Procedure, (735 ILCS 5/1-109 (West 2012)), entitled "Verification by certification," a Class 3 felony, when he filed with the Circuit Court of Cook County his Response to Amended Petition for Order of Protection and Motion to Strike and Dismiss in In re the Parentage of [redacted] a Minor Child, by Sanchez v. [redacted], No. 2015 D [redacted], in which he averred that he "has never threatened [redacted] nor her family," a false statement material to an issue or point in question that he did not believe to be true but that he certified as true and correct pursuant to section 1-109 of the Illinois Code of Civil Procedure.

From about March 2012 through about January 2015, or on one or more dates therein, Officer Sanchez violated section 12-7.5(a-3)(1) of the Illinois Criminal Code of 2012 (720 ILCS 5/12-7.5(a-3)(1) (West 2012)), entitled "Cyberstalking," when he knowingly and without lawful justification harassed [redacted] on at least two separate occasions through the use of electronic communication in the form of text and/or instant messages that included threats of immediate or future bodily harm directed towards [redacted] including, but not limited to, [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] or words to that effect.

From about March 2012 through about January 2015, or on one or more dates therein, Officer Sanchez violated section 12-7.5(a)(1) and/or section 12-7.5(a)(2) of the Illinois Criminal Code of 2012, (720 ILCS 5/12-7.5(a)(1)-(2) (West 2012)), entitled "Cyberstalking," when he engaged in a course of conduct using electronic communication directed at [redacted] in the form of text and/or instant messages, including, but not limited to, [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] or words to that effect, and/or a digital photograph of Officer Sanchez's palm and fingers in the shape of a gun with the residential building of an individual associated and/or previously associated with [redacted] in the background, that he knew or should have known would cause a reasonable person to fear for her safety and/or the safety of a third person and/or suffer other emotional distress.

From about March 2012 through about January 2015, or on one or more dates therein, Officer Sanchez violated section 26.5-3(a)(5) of the Illinois Criminal Code of 2012, (720 ILCS 5/26.5-3(a)(5) (West 2012)), entitled "Harassment through electronic communications," when he used electronic communication in the form of text and/or instant messages, including, but not limited to, [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] and/or [redacted] or words to that effect, and/or a digital photograph of Officer Sanchez's palm and fingers in the shape of a gun with the residential building of an individual associated and/or previously associated with [redacted] in the background, to threaten injury to [redacted] and/or to any of her family and/or household members.
