Charles A. Nutall

On or about December 9, 2000, inside the Timothy J. O'Connor Education Training Division, located at 1300 West Jackson, Civilian Charles A. Nutall, committed the offense of forgery, in that he knowingly submitted a false Triton College Transcript to the department, which had been altered by him in such a manner as to indicate that he had earned approximately 44 hours of college credit at Triton College, when, in fact, he had earned approximately 9 hours of college credit at Triton College, thereby violating Chapter 720 ILCS 5/17-3.

On or about December 9, 2000, inside the Timothy J. O'Connor Education Training Division, located at 1300 West Jackson, Civilian Charles A. Nutall, committed the offense of unlawful practices in employment applications, in that he knowingly signed and submitted a false Personal History Questionnaire/Background Investigation/City of Chicago form to the department, indicating that he had earned approximately 44 hours of college credit at Triton College, when, in fact, he had earned approximately 9 hours of college credit at Triton College, thereby violating, City of Chicago Municipal Code, Chapter 2-74, Section 095.

On or about December 9, 2000, inside the Timothy J. O'Connor Education Training Division, located at 1300 West Jackson, Civilian Charles A.vNutall, knowingly submitted a false Triton College Transcript to the department, which had been altered by him in such a manner as to indicate that he had earned approximately 44 hours of college credit at Triton College, when, in fact, he had earned approximately 9 hours of college credit at Triton College, thereby bringing discredit upon the department and impeding the department's efforts to achieve its policy and goals.

On or about December 9, 2000, inside the Timothy J. O'Connor Education Training Division, located at 1300 West Jackson, Civilian Charles A. Nutall, knowingly signed and submitted a false Personal History Questionnaire/Background Investigation/City of Chicago form to the department, indicating that he had earned approximately 44 hours ofncollege credit at Triton College, when, in fact, he had earned approximately 9 hours of college credit at Triton College, thereby bringing discredit upon the department and impeding the department's efforts to achieve its policy and goals.

Nutall made a false report, written or oral, when he knowingly signed and submitted a false Personal History Questionnaire/ Background Investigation/City of Chicago form to the department, indicating that he had earned approximately 44 hours of college credit at Triton College, when, in fact, he had earned approximately 9 hours of college credit at Triton College.
