Golich operated Department motor vehicles and did not have a valid Illinois driver's license in violation of General Orders 81-8, Addendum IA, Article III, Section A-2 (rescinded and renumbered as 97-2, Addendum 1, Article III, Section A-1-3, May 1, 1997.)

That from at least July 30, 1990, Officer James J. Golich failed to immediately submit a report to his commanding officer regarding the revocation of his Illinois driver's license on December 17, 1989, and November 27, 1997, thereby violating General Orders 81-8, Addendum IA, Article III, Section A-3 (rescinded and renumbered as 97-2, Addendum 1, Article III, Section A-1-f.)

That from at least July 30, 1990, Officer James J. Golich did not possess a valid Illinois driver's license while on duty, thereby violating General Order 88-15, Addendum 2D, Article X, Sections A and L(6).

That between December 17, 1989, and June 17, 1997, Officer James J. Golich drove a motor vehicle while his driver's license was suspended and/or revoked (including, but not limited to the following dates: April 14, 1998, February 28, 1997, and June 12, 1997), in violation of 625 ILCS 5/6-603(a).

That on November 18, 1996, Officer James J. Golich represented to his commanding officer the driver's license issued to his brother, Jacob, as his own, in violation of 625 ILCS 5/6-301(a)3.
