From on or about January 14, 1998, to the present, or for some period of time therein, Police Officer Mark C. Macklin failed to provide the Department with his current residence information.

From on or about January 14, 1998, to the present, or for some period of time therein, Police Officer Mark C. Macklin failed to submit a Personnel Change Notice and Change of Address form to the Chicago Police Department with regard to his change of address and telephone number in violation of General Order 94-5, Addendum 8, Article II, Section A 1 and 2, and General Order 94-5, Addendum 8, Article III, Section C.

The Superintendent did not prove by a preponderance of the evidence that from on or about January 14, 1998, to the present, or for some period of time therein, Police Officer Mark C. Macklin has resided at 16511 Evans Avenue, South Holland, Illinois, thereby failing to actually reside within the corporate boundaries of the City of Chicago.

The Superintendent did not prove by a preponderance of the evidence that from on or about January 14, 1998, to the present, or for some period of time therein, Police Officer Mark C. Macklin failed to reside within the corporate boundaries of the City of Chicago by residing at 16511 Evans Avenue, South Holland, Illinois, thereby impeding the Department's efforts to achieve its policy and goals and/or bringing discredit upon the Department.

The superintendent did not prove by a preponderance of the evidence that from on or about January 14, 1998, to the present, or for some period of time therein, Police Officer Mark C. Macklin failed to reside within the corporate boundaries of the City of Chicago by residing at 16511 Evans Avenue, South Holland, Illinois, thereby failing to be an actual resident of the City of Chicago, in violation of the City of Chicago Municipal Code, Chapter 2-152, Section 340.
