Count 1 On or about December 29, 2015, Police Officer Carol Weingart falsely stated to Assistant State's Attorney Nicholas Kantas that she was in front of Giovanni Valle's vehicle and/or that Valle's vehicle came toward her, or words to that effect. Count 2 On or about December 29, 2015, Police Officer Carol Weingart falsely stated to Assistant State's Attorney Nicholas Kantas that she was in front of Giovanni Valle's vehicle and/or that Valle's vehicle came toward her, or words to that effect. Count 3 On or about December 29, 2015, Police Officer Carol Weingart falsely stated to Sergeant Angel Perez that Giovanni Valle attempted to strike her with his vehicle, or words to that effect. Count 4 On or about December 29, 2015, Police Officer Carol Weingart falsely stated to Detective Greg Jacobson that she was in the northbound lane of traffic (at or near 3028 N. Clybourn) prior to Officer Laura Kuhlmann discharging her weapon at Giovanni Valle's moving vehicle, and/or that Valle's vehicle was coming toward her (Officer Weingart) and she (Officer Weingart) ran to avoid being struck, or words to that effect.

Count 1 On or about January 13, 2016, at the offices of the Independent Police Review Authority("IPRA"), Police Officer Carol Weingart told IPRA Investigator Thersa Davis that on or about December 29, 2015, she was in the lane of the offender's car and/or the oncoming lane of traffic prior to Officer Laura Kuhlmann discharging her weapon at Giovanni Valle's vehicle, or words to that effect. Count 2 On or about December 29, 2015, Police Officer Carol Weingart falsely stated to Assistant State's Attorney Nicholas Kantas that she was in front of Giovanni Valle's vehicle and/or that Valle's vehicle came toward her, or words to that effect. Count 3 On or about December 29, 2015, Police Officer Carol Weingart falsely stated to Sergeant Angel Perez that Giovanni Valle attempted to strike her with his vehicle, or words to that effect.
