Rule 2	3 counts	Any action or conduct which impedes the Department's efforts to achieve its policy and goals or brings discredit upon the Department. Count 1 On or about August 25, 2014, and during a recorded statement given to the Independent Police Review Authority ("IRPA"), Officer Sampim falsely stated that on or about August 24, 2014, at or near 2842 West Polk Street, Chicago, illinois, he saw "a male black in a dark shirt with his arm extending with a silver pistol," or words to that effect, prior to Police Officer Robert Slechter ("Officer Slechter") shooting Roshad McIntosh ("McIntosh"). Count 2 On or about August 3, 2015, in Defendant Officer Sampim's Answer to Plaintiff's First Set of Interrogatories to Defendants produced in the matter of Estate of Roshad McIntosh v. City of Chicago, et al., Case No. 15 CV 01920 (N.D. III. filed Mar. 4, 2015), Officer Sampim falsely stated that on August 24, 2014, he "observed McIntosh in the backyard area with a gun in his hand, pointing it at Officer Slechter," prior to Officer Slechter shooting McIntosh. Count 3 On or about January 13, 2016, and while being deposed under oath in the matter of Estate of Roshad McIntosh v. City of Chicago, et al., Case No. 15 CV 01920 (N.D. III. filed Mar. 4, 2015), Officer Sampim knowingly made a false statement which he believed was not true at the time when he made it and which was material to the issue or point in question when he stated that on or about AUgust 24, 2014, at or near 2842 West Polk Street, Chicago, Illinois, he "saw Roshad McIntosh with his handgun in his right hand, pointing it to Officer Slechter," or words to that effect, prior to Officer Slechter shooting McIntosh.
