On or about August 11, 2001, at or near 7924 South Kildare, Chicago, Illinois, Officer Uginchus, while off duty and without lawful authority, approached 12-year-old Matthew Basile in a menacing manner and/or flicked cigarette ash and/or a lighted cigarette -in Basile's direction, placing him in reasonable apprehension of receiving a battery, thus committing aggravated assault in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-2(a)(1).

On or about August 11, 2001, at or near 7924 South Kildare, Chicago, Illinois, Officer Uginchus, while off duty and without lawful authority, approached Bridget Basile in a menacing manner and/or brandished garden shears, placing her in reasonable apprehension of receiving a battery, thus committing aggravated assault in violation of Illinois Compiled Statutes chapter 720 ILCS 5/12-2(a)(1).

On or about September 13, 2001, at or near 7918 South Kildare,Chicago, Illinois, Officer Uginchus, knowingly transmitted tb a police officer a report of an assault which he knew was groundless, thus
engaging in disorderly conduct in violation of Illinois Compiled Statutes chapter 720 ILCS 5/26-1(a)(4).

On or about September 13, 2001, at or near 7918 South Kildare, Chicago, Illinois, Officer Uginchus, knowingly transmitted to a sergeant of police a report accusing a neighbor of causing damage to his property which he knew was groundless, thus engaging in disorderly conduct in violation of Illinois Compiled Statutes chapter 720 ILCS 5/26-1(a)(4).

On or about September 21, 2001, Officer Uginchus appeared at the offices of the Internal Affairs Division to provide a formal statement to pending charges, and during that statement he admitted that he had purchased and possessed a Smith & Wesson .9 mm handgun, serial number VDL2482, which was not registered, thus violating City of Chicago Municipal Code, Chapter 8-20, Section 040.
