On or about August 25, 2001, while on duty and in the vicinity of a Dominick's food store located at or near 2101 East 71st Street, Chicago, Illinois, after learning that a bank robbery was in progress or had just been committed, she failed to take the appropriate police action, including, but not limited to: making the required notifications to the Department; attempting to arrest the offender(s); preserving the crime scene for evidence collection and processing; locating, identifying and/or i nterviewing w itnesses; obtaining a description o f the offender(s); identifying herself and providing information to police units arriving on the scene; and /or notifying the Department prior to leaving the scene.

On or about December 10, 2001, while in the vicinity of 3510 South Michigan Avenue, Chicago, Illinois, she gave a statement to the Internal Affairs Division that contained false information.

On or about August 25, 2001, at approximately 1245 hours, while on duty and in the vicinity of a Dominick's food store, located at or near 2101 East 71st Street, Chicago, Illinois, after learning a bank robbery was in progress or had just been committed, she failed to take appropriate police action, including, but not limited to making the required notifications to the Department; attempting to arrest the offender(s); preserving the crime scene for evidence collection and processing; locating, identifying and/or interviewing witnesses; obtaining a description of the offender(s); identifying herself and providing information to p olice units arriving on the scene; and /or notifying the Department prior to leaving the scene.

On or about December 10, 2001, while in the vicinity of 3510 South Michigan Avenue, Chicago, Illinois, she gave a statement containing false information to the Internal Affairs Division.

On or about August 25, 2001, at approximately 1245 hours, while on duty and in the vicinity of a Dominick's food store located at or near 2101 East 71st Street, Chicago, Illinois, after learning that a bank robbery was in progress or had just been committed, she failed to report promptly to the Department any information concerning the robbery
